Majority of mortgage companies especially VA Loans require a NPMA-33 WDI form. A  inspection report for that is only used to report activity or damage from wood destroying insects that, for the purposes of this report, include termites, carpenter ants, carpenter bees, and reinfesting wood boring beetles.
